A Case of Terminal Deoxynucleotidyl Transferase Positive Acute Myeloblastic Leukemia (FAB type M2)

Abstract

This case was Auer -rod positive acute myeloblastic leukemia¢¥ with maturation (FAB type M2) and more than 50 % of blasts showed terminal deoxynucleotidyl transferase (TdT) positivity by indirect immunoperoxidase procedure. More than 20 % of blasts were positive for myeloperoxidase(MPO), Sudan Black B(SBB), and chloroacetate esterase. Less than 1 % of blasts were positive for periodic acid-Schiff(PAS) and leukemic cells were negative for alpha-naphthyl butyrate esterase. The cytochemical staining results showed no diffference from general finding of TdT negative AML.
Immunophenotyping performed using an indirect immunoperoxidase procedure with a panel of myeloid lineage monoclonal antibodies. More than 20% of blasts were positive for CDl lc (Leu-M5 ), CD13 (My7 ), CD14 (My4 ), CD15 (Leu-Ml ), and HLA-DR. Most of blasts were negative for CD33(My9) while the positivity of AML has been reported. The intensity of immunostaining was shown weak to strong for CD11c(Leu-M5), weak for CD13(My7), moderate for CD14(My4), strong for CD15(Leu-Ml) and HLA-DR in blasts.
